Triage Nurse

Regional Hospice & Palliative Care (RHPC) is seeking a licensed registered nurse to join the triage team and support the clinical supervisor and hospice team in the coordination of service delivery to Regional Hospice patients and families. The Triage Nurse works primarily from the office to provide clinical and administrative direction to field staff, and uses critical thinking skills to assess and assists with solving patient needs.

Qualifications:

  • A current license to practice as a Registered Nurse in the State of Connecticut. Certification in Hospice and Palliative Care (CHPN) preferred.
  • Minimum two (2) years of experience in a fast-paced healthcare environment required as well as experience in communicating with patients, families and other members of the healthcare community.
  • Minimum two (2) years home health or hospice coordination/administrative experience preferred.

Essential Functions:

  • Supports the coordination of clinical operations of the agency, ensuring quality of care.
  • Serves as first point of contact for issues related to patient care from patients, families, physician’s offices, facilities and others including morning report from overnight On-Call RN.
  • Works assigned shifts of On-Call to meet the needs of the agency.
  • Stays abreast of best nursing practice standards for pain and symptom management.
  • Collaborates with Director of Counseling Services to resolve issues and better coordinate agency patient care services.
  • Ensures adherence to the RHPC Compliance Program.
  • Collaborates in the maintenance of medical records to ensure accuracy and compliance with regulations and the HIPAA Privacy Rule.
  • Participates in self-evaluation and professional development.

This position may involving driving an automobile varied distances to patient homes.

The Triage Nurse must possess good communication and organizational skills and be able to work in a variety of environments. The Triage Nurse presents a professional attitude and appearance at all times while maintaining strict confidentiality of patient and agency information.

This position interfaces with patient, families, agency interdisciplinary personnel, office staff, clerical staff, hospital staff, hospital based and community-based physicians, and the public.
