A Doctor’s Referral is Not Necessary

Our hospice care is open to all, and anyone can request our services. Some patients are referred to us by friends, family members, or other medical facilities and will be admitted to us under a physician’s direction. We also have an online hospice referral that can be filled out and completed on our website.

Hospice is Affordable

Hospice care is covered by Medicare, Medicaid, and a majority of private health insurances, which allows you to worry about your loved one and not about any financial issues.

Perinatal & Pediatric Care

We support all of our Brookfield patients throughout their journey with us. Our Pediatric & Perinatal Hospice Program focuses on a multitude of services including physical, emotional, and spiritual support during this difficult time. For families whose babies or children have been diagnosed with a terminal illness, or have passed, on we can offer our care from pregnancy to remembrance.

Our Pediatric & Perinatal Hospice care involves visits from dedicated healthcare professionals who have experience in perinatal care. They can help care for babies at home and can connect you and your family with community resources in the Brookfield area.
