A Doctor’s Referral is Not Necessary
There are a variety of people and facilities that can refer your loved one to receive hospice care, including: hospitals, clergy and family members, friends, and staff members at other home care agencies. Our patients are admitted under a physician’s direction, and for those looking for a different referral source we offer a hospice form you can fill out online to request care.

Hospice is Affordable
Medicare, Medicaid and most private health insurances cover hospice care, reducing some of the financial burden that comes along with end-of-life services, and provides your loved one with quality care.

Perinatal & Pediatric Care

With our Pediatric & Perinatal Hospice Program, Hartford families can seek out physical, emotional, and spiritual support to help them cope with their babies terminal condition or death. Throughout your journey we will provide medical support and psychological guidance from pregnancy to remembrance.

Pediatric & Perinatal Hospice care includes visits by healthcare professionals who specialize in perinatal care, and can guide families on how to care for their baby at home. They will also assist you in connecting with community resources for further care.
