Physicians, registered nurses, social workers, chaplains, hospice care aides and trained hospice volunteers follow care plans individually designed for each patient. A Doctor’s Referral is Not Necessary
Anyone may request hospice care, we offer patients an online referral form for their convenience. A patient is typically referred by a hospital, clergy, family, friend or other home care agency and will be admitted under a physician’s orders.
Hospice is Affordable
Hospice care is covered by Medicare, Medicaid and most private health insurances, letting you and your family focus on providing expert care for your loved one.
Home Hospice Care allows you to stay in the comfort of your Oakville home, nursing home, or assisted living facility.

For our patients who are currently living in an assisted living facility, we will create a contract through Regional Hospice to monitor the patients development. This normally involves trained and trusted volunteers to help our local caregivers.

At our Inpatient Hospice Center in Danbury we accommodate our patients and their families while providing top medical care and respecting each patient’s goals.
