Physicians, registered nurses, social workers, chaplains, hospice care aides and trained hospice volunteers follow care plans individually designed for each patient. We focus on the individuals symptoms, condition, and pain levels to create a plan of action. We may also suggest certain therapies like speech, physical and occupational.

Comfort Versus Curative
After a terminal prognosis of 6 months or less has been made, or if a child is suffering from a life-limiting illness, our hospice services should be considered. We are here to help both the patient and their family through this difficult time.
A Doctor’s Referral is Not Necessary
Anyone may request hospice care, we offer patients an online referral form for their convenience. A patient is typically referred by a hospital, clergy, family, friend or other home care agency and will be admitted under a physician’s orders.

Hospice care is covered by Medicare, Medicaid and most private health insurances, letting you and your family focus on providing expert care for your loved one.

For our patients who are currently living in an assisted living facility, we will create a contract through Regional Hospice and Palliative Care to monitor the patients development. This normally involves trained and trusted volunteers to help our local caregivers.
