Compassion, Peace and Dignity
By supporting our patients, providing reputable medical care, and creating lasting bonds between patient and caregiver, our hospice care is truly unbeatable. We put our patients needs above all else, tailoring treatment plans and respecting their personal wishes.
Comfort Versus Curative
We can provide our reputable hospice care when a terminal prognosis of 6 months or less is determined, or when a child is battling the effects of a life-limiting illness. We care for each patient with love and respect.
A Doctor’s Referral is Not Necessary
Anyone in need of end-of-life care can request our hospice care. A doctor’s referral is not required, though patients will be admitted under a physician. Friends, family, agencies, or hospitals can refer patients to us, we even have an online referral form for easier access.
Hospice is Affordable
Hospice should be affordable for all. That’s why our quality hospice care is covered by Medicare, Medicaid, and most private health insurance companies.
Our quality Home Hospice Care comes to you whether you live at home in West Haven, an assisted living facility, or skilled nursing home.
